Unlimited messaging & support between visits with your Registered Dietitian & Integrative nutritionist on navigating challenges, lab work questions and vitamin/mineral changes/recommendations.

Please read through the following payment plans before completing your patient application and having a discovery call with our Registered Dietitians.

Membership package

 We are a membership based practice which allows unlimited care in between scheduled visits.

​

Memberships can be paid annually or in 3 consecutive payments.

 Package price adjustments are made after insurance coverage is applied. HSA/FSA accepted.

​

Pricing varies from $1,050-$3,060/year based on insurance coverage. After 1 year of membership, members are offered a discounted membership and per visit fee schedule. 

​

Insurance benefits may be applied to pay for a portion of the membership if in-network benefits are provided and confirmed.

Eating Disorder Care:

The following membership is for all patients being seen for eating disorder care. 

​

Insurance benefits apply to 1 to 1 visits only. This is does not include required monthly membership fees.

​

Insurance coverage is not guaranteed & must be confirmed by the patient. Package price adjustments are made after insurance coverage is applied.

 HSA/FSA accepted.

Additional Sessions

Most patients benefit from 2 sessions/month. After the 7 sessions in the membership are utilized, members have the opportunity to purchase 1 to 1 sessions per visit.

 Pricing per follow up session after 7 visits are met:

 

Erin Falco, RDN: $240

Veronica Rechten, MSc: $180

Raye McDonald, MS:  $180

Can I Use My Insurance?

​

We are in-network with select insurance providers. To confirm your insurance benefits of coverage for medical nutrition therapy. Please refer to our In-Network Insurance Guide. 

​

Cancellation Policy

Commitment to your care must be mutual. We value both our practitioner's and patient's time and have implemented this policy for this reason. When you miss an appointment with us, we are not able to provide the most comprehensive care AND lose the opportunity to see other patients who could have scheduled an appointment for the same time or are on a waiting list to be seen. We do not operate like a traditional Drs office as our appointments are lengthy and we spend time before your scheduled appointment reviewing your case.

 

We have a strict 72 hour cancellation policy. A fee of $360/$420 for an initial appointment or $180/$240 for follow up appointments will be charged to the card on file. This applies to no shows and short notice cancellations/ rescheduling notices. If you would like to cancel or reschedule, you must contact our office via email to admin@roothealthnj.com or leave a voicemail at 732-784-8378.

Without a medical Dr's note, You will be charged the fee of the whole appointment cost or a session if not cancelling within the appropriate time frame. This does not get billed to insurance.
